There are a variety of services offered at Sylvan Family Health Centre that are not covered by your Alberta Health Care Insurance Plan.

Our Clinic Policy in regards to the completion of insurance papers or other forms that require an Attending Physician’s Statement is as follows:
Physicians at Sylvan Family Health Centre will complete forms within 6 weeks of receipt from patient or third party.
