To the Sylvan Family Health Centre Patient Community,

 

As I am sure you are aware there has been many changes to health care delivery in Alberta over the past years and this has created many personal and professional challenges delivering primary care in Alberta. Through much thoughtful discussion with my immediate family, I have come to the decision to close my clinical practice.  This difficult decision was not made lightly, and it saddens me that I will not be able to continue with you on your health journey.

 

Fortunately, the clinic is in a good position to support this transition, and each of you will still be cared for by the excellent team at Sylvan Family Health Centre.  While you may be losing me as your primary provider, you will not be losing your medical home.  The clinic is aiming to recruit to the team, but there is no one yet slotted to replace me. My physician colleagues and health team at SFHC will do everything they can to absorb and care for my panel until a recruit can be secured. If no recruit can be found by Spring 2027, and you have not landed with a new provider, they may not be able to care for you long term and you may need to find a new provider elsewhere.

 

Several patients will be selected at random to be paneled with a new Provider and will be contacted.

If you have any questions about how this will affect your access to care, please feel free to contact the clinic at 403-887-2224. Again, I will stress that all of you, should you choose to stay, you will continue to receive high-quality care at Sylvan Family Health Centre in my absence.

 

I would like to extend my warmest thanks to you, my patients, for trusting me to help care for your most valuable asset – your health – over these past 5 years.  It has been an honor and a privilege to walk alongside you and provide assistance to you where I was able to do so.
