In a Primary Care Network (PCN), a group of family doctors works with Alberta Health Services and other health professionals to coordinate the delivery of team based comprehensive services for their patients. Sylvan Family Health Centre is a member of the Wolf Creek PCN. This collaboration allows us to offer comprehensive services to our patients that are specific to the needs of Sylvan Lake. The health care professionals that we currently have working alongside us include:

• Mental Health Consultants

• PCN Licensed Professional Nurse and Registered Nurse

There are several free workshops available for our patients through the PCN. These can include:

• Happiness basics

• Anxiety to Calm
