When patients receive health care services from Sylvan Family Health Centre, we collect individually identifying personal & health information and share this internally as well as with other health service providers that are involved in your individual care.

Your personal health information is collected, used and disclosed in accordance with the provisions of the Health Information Act. The primary use of your health information is to provide diagnostic and treatment services to you, our patient. It is also used to bill Alberta Health and Wellness for the services provided.
